Please take the time to read and understand the following information and policies before we groom your pet:

• Grooming can sometimes expose or aggravate a hidden pre-existing condition. We cannot diagnose a condition, but may advise you to seek veterinary attention. If your pet has any medical problems (e.g. seizures, arthritis, collapsing trachea), please make us aware of them so that we can take appropriate steps to keep your pet comfortable.

• If fleas are found on your pet(s), we will attempt to notify you and we will give a flea bath automatically. There is an extra charge for this service, depending on how much time is required to remove the fleas from your pet and our salon. The cost covers special shampoo that kills the fleas on your pet and the extra clean-up required to ensure that no fleas are active in the salon. Please note that, if fleas are in your home or pet area, the shampoo will not protect your pet(s) once they leave our salon. With your permission, an oral flea treatment may be administered in extreme cases.

• If your pet has any history of biting, we require notification in order to prevent injury to our staff and your pet(s). If your pet has behavior issues, we will be happy to do what we can to help work through them.

• If your pet has to be “shaved down,” it has been our experience that pre-existing conditions may be revealed. When a pet is very matted, air cannot reach its skin and the pet may itch and bite itself, causing the skin to become sore and unable to heal. Shaving uncovers these skin conditions, as well as other problems that may have developed from the lack of air and grooming. Please note that it is difficult to shave down a matted pet without slightly irritating the skin. WE WILL NOT SHAVE YOUR PET WITHOUT YOUR CONSENT. There will be an extra charge for this service, if you agree to it.

• Cancellations happen and we understand schedules sometimes change. If you must cancel, we ask that you give as much notice as possible. Chronic cancellation may result in the inability to book future appointments and a fee up to the amount of the grooming.

• If you do not show up for your appointment, you may be subject to a fee. Habitual no-shows may be subject to a fee up to the amount of the grooming and may no longer be able to book future appointments.

• THERE IS A $25 FEE FOR RETURNED CHECKS.

• We reserve the right to refuse service to anyone whose pet(s) may pose a threat to our employees or other pets left in our care, whether due to aggression, health problems, or parasites other than fleas.

• Although accidents are very rare, there is a risk when working with pets and sharp objects. We will inform you immediately if any incident occurs, or if we notice something unusual. Your pet’s safety and comfort is our number one priority.

By entering your name and today’s date below, you agree that you have read and understood the above information and policies.

You further agree to release and hold harmless Muddy Paws, its owners, agents, employees, and representatives, from and against any and all liabilities, claims, actions, suits, charges, and judgments arising from any services provided, including injury to and death of your pet(s), whether such occurrence happens in our care or after services have been rendered.

You further attest that your pet(s) are current on all vaccinations.

You further authorize us to transport your pet(s) to a veterinarian, at our sole discretion, and agree to assume full responsibility for any resulting veterinary bills, including those related to your pet(s) being matted, senior, or having pre-existing or undiagnosed conditions. Safety is our primary concern.
